Terms and Conditions
1. Introduction
These Terms and Conditions ("Terms") govern the relationship between Quality Homemakers, Inc. ("Company," "we," "our," or "us") and you ("Client," "you," or "your") concerning the provision of home care services. By engaging our services, you agree to comply with and be bound by these Terms.
2. Services Provided
Quality Homemakers, Inc. offers a range of home care services, including:
Personal Care Assistance:
Support with activities of daily living such as bathing, dressing, and grooming.
Live-In and 24/7 Support:
Continuous care to ensure safety and comfort at home.
Homemaking Services:
Assistance with household tasks like cleaning, meal preparation, and laundry.
Companionship:
Providing social interaction and emotional support.
The specific services provided will be detailed in a separate Service Agreement tailored to your individual needs.
3. Eligibility and Assessment
To be eligible for our services, you must:
Be a resident of Connecticut.
Require assistance with activities of daily living.
Undergo an initial assessment conducted by our qualified staff to determine the appropriate level of care.
The assessment will evaluate your medical, functional, and psychosocial status to develop a personalized care plan.
4. Service Agreement
Upon completion of the assessment, a Service Agreement will be drafted, outlining:
The specific services to be provided.
The schedule and duration of services.
The fees associated with the services.
Any additional terms pertinent to your care.
Both parties must sign the Service Agreement before services commence.
5. Fees and Payment
Fees for services are determined based on the care plan developed during the assessment. Payment terms include:
Invoices issued bi-weekly.
Payment due within 14 days of invoice date.
Acceptable payment methods: check, credit card, or electronic funds transfer.
Late payments may incur additional charges as specified in the Service Agreement.
6. Client Responsibilities
As a Client, you agree to:
Provide accurate and complete information during the assessment.
Notify us promptly of any changes in your health status or care needs.
Ensure a safe environment for our staff to perform their duties.
Comply with the care plan and any recommendations made by our staff.
7. Company Responsibilities
Quality Homemakers, Inc. commits to:
Delivering services as outlined in the Service Agreement.
Employing qualified and trained staff.
Maintaining confidentiality of your personal and medical information.
Complying with all applicable federal and state regulations, including the Connecticut Home Care Program for Elders (CHCPE) guidelines.
8. Confidentiality and Privacy
We are committed to protecting your privacy. All personal and medical information collected will be used solely for providing care and will not be disclosed to third parties without your consent, except as required by law.
9. Termination of Services
Either party may terminate the Service Agreement with a 14-day written notice. Immediate termination may occur if:
The Client fails to comply with the Terms and Conditions.
The Client's care needs exceed the scope of services we can provide.
Safety concerns arise that cannot be mitigated.
10. Limitation of Liability
While we strive to provide high-quality care, Quality Homemakers, Inc. is not liable for:
Any injury or harm resulting from the Client's failure to follow medical advice.
Loss or damage to personal property.
Any indirect, incidental, or consequential damages arising from our services.
11. Governing Law
These Terms and Conditions are governed by the laws of the State of Connecticut. Any disputes arising from these terms shall be resolved in the courts of Connecticut.
12. Amendments
Quality Homemakers, Inc. reserves the right to amend these terms and conditions. Clients will be notified of any changes, and continued use of our services constitutes acceptance of the revised terms.
